This proposed Risk Management Process Notification and tracking form is designed to facilitate the risk assessment process (ISO 31000) as outlined in the diagram below.

• This form is to be used for the notification and monitoring of risks.

• The form should be used for the initial identification and analysis of risk, including proposals for treatment (mitigation etc.).

• The forms completion should result in an entry in one of the two risk registers maintained by CIHE – the Risk Register and the Academic Risk Register.

• The updated form can then be used for discussion at subsequent meetings – it makes a handy document for tabling and discussing.
